Ovฤa Protocols ยท PMOS Edition
Your body isn't broken.
It's been misunderstood.
"I've been diagnosed with PMOS (PCOS) and handed a pamphlet. It felt so impersonal."
A note on the name
PMOS โ Polycystic Metabolic Ovarian Syndrome โ is the updated name for the condition previously known as PMOS (PCOS) (Polycystic Ovary Syndrome). The name change reflects a growing clinical consensus that this is fundamentally a metabolic syndrome with hormonal consequences, not simply an ovarian or reproductive condition. You may still see PMOS (PCOS) used by your provider, in your records, and across most medical literature โ both names refer to the same diagnosis.
Throughout this guide, PMOS is used as the primary term with PMOS (PCOS) noted in parentheses where relevant.
PMOS (formerly PCOS) is one of the most common hormonal conditions in women of reproductive age, and one of the most under-explained. This protocol was built for women who are ready to move past the diagnosis and into real, evidence-based action. Every recommendation here is grounded in clinical research and chosen with 19 years of reproductive imaging experience behind it.
โ Educational resource only. Not medical advice. All supplementation should be discussed with your licensed healthcare provider before beginning. This protocol does not replace clinical care or diagnosis.
PMOS (Polycystic Metabolic Ovarian Syndrome) is a metabolic condition.
Not just a hormone problem.
The most important thing to understand about PMOS (PCOS) is that it is not one single condition โ it is a syndrome with multiple root causes. Insulin resistance, androgen excess, chronic inflammation, HPA axis dysregulation, and thyroid dysfunction all play a role, and they show up differently in every woman. This protocol is organised by root cause so you can identify which drivers matter most for your body โ and target them directly.
Section 01 ยท Foundation
Start here. Every time.
Without exception.
"I've been taking random supplements for years. Where do I actually begin?"
Before anything else, every woman with PMOS (PCOS) needs a solid nutritional foundation. These are the non-negotiables โ the nutrients that stabilise the hormonal environment, support the adrenal and thyroid systems, and prepare the body for everything else in this protocol to work properly.
The foundation every PMOS (PCOS) protocol is built on โ and the one most women are getting wrong
Why it matters
Up to 40% of women carry a gene variant (MTHFR) that makes it difficult to convert standard folic acid into the active form the body can use. For women with PMOS (PCOS) โ who already carry elevated risk for metabolic and cardiovascular complications โ this matters enormously. A methylated multivitamin delivers active folate (L-methylfolate), active B12 (methylcobalamin), and active B6 (P-5-P) already converted and ready for your cells. These B vitamins are critical for hormone metabolism, homocysteine regulation, and the neurotransmitter balance that underpins mood and stress resilience in PMOS (PCOS).
What to look for
Check the label for "L-methylfolate" or "5-MTHF" โ not "folic acid." Check for "methylcobalamin" โ not "cyanocobalamin." These are the two most important active form indicators. A quality women's multi for PMOS (PCOS) should also contain iron (in a gentle bisglycinate form if possible), magnesium, zinc, and selenium without exceeding safe upper limits on any single nutrient.
Deficient in up to 85% of women with PMOS (PCOS) โ and directly linked to insulin resistance and ovulatory dysfunction
Why it matters
Vitamin D is not just a bone nutrient. In women with PMOS (PCOS), Vitamin D receptors are present in the ovaries and on insulin-producing pancreatic cells โ and deficiency is associated with worsened insulin resistance, irregular ovulation, higher androgen levels, and increased inflammatory markers. Correcting deficiency is one of the highest-yield interventions in PMOS (PCOS) management, and it is one of the most consistently under-addressed.
Dosage guidance
Ask your provider for a 25-OH Vitamin D blood test before dosing. Target serum level: 50โ80 ng/mL. Most women with PMOS (PCOS) need 3,000โ5,000 IU daily to reach and maintain this range. Pair with K2 MK-7 (100โ180mcg) โ this ensures calcium is directed to bone rather than soft tissue at higher D3 doses. Take with a fat-containing meal for optimal absorption.
Section 02 ยท Insulin Resistance & Blood Sugar
This is the root of most PMOS (PCOS).
And it is treatable.
"I was told my blood sugar is 'fine' โ but no one explained why I still gain weight, can't ovulate, and feel exhausted after eating."
Insulin resistance is present in up to 70% of women with PMOS (PCOS) โ including lean women with normal blood glucose. It drives elevated androgens, disrupts follicle development, promotes fat storage (especially around the abdomen), worsens inflammation, and makes ovulation unpredictable. Addressing insulin sensitivity is the most impactful single intervention available for most women with PMOS (PCOS) โ and it starts before any fertility treatment.
The single most evidence-backed supplement for PMOS (PCOS) โ start here before anything else in this section
Why it matters
Inositol acts as a second messenger in the insulin signalling pathway. Women with PMOS (PCOS) are frequently deficient in it at the cellular level โ not because they don't consume enough, but because of a defect in how their tissues handle it. This causes a cascade effect: insulin resistance worsens, FSH signalling becomes impaired, follicle development stalls, and androgen levels rise. The 40:1 Myo:D-Chiro ratio mirrors the physiologic ratio found in healthy ovarian follicular fluid and has been shown to outperform higher D-Chiro ratios in most women.
What I look for on imaging
On pelvic ultrasound, classic PMOS (PCOS) morphology shows multiple small follicles (2โ9mm) arranged around the periphery of the ovary โ the "string of pearls" pattern โ with increased ovarian volume (above 10cc) and often a thickened, echogenic (brighter) central stroma. When I see this picture alongside your symptom history, inositol at the 40:1 ratio is the very first protocol I recommend. Allow 3โ6 months for full clinical effect.
A botanically derived insulin sensitiser with the most robust evidence base of any PMOS (PCOS) botanical
Why it matters
Berberine works by activating AMPK โ the same cellular energy-sensing pathway that metformin targets โ which improves insulin sensitivity, reduces glucose production in the liver, and modulates gut microbiome composition in ways that further support blood sugar regulation. In women with PMOS (PCOS), berberine has been shown to reduce fasting insulin, lower testosterone, reduce LH:FSH ratio, and improve ovulation rates. It is one of the few botanicals that matches pharmaceutical intervention in clinical trials.
Important notes
Berberine significantly affects drug metabolism via CYP3A4 inhibition โ it should not be combined with hormonal contraceptives, metformin, thyroid medication, or blood pressure medications without provider guidance. It also has antimicrobial properties and should be cycled (8 weeks on, 4 weeks off) to protect gut microbiome diversity. Not appropriate during pregnancy or when trying to conceive without specialist guidance.
A trace mineral that makes insulin work more efficiently at the cellular level
Why it matters
Chromium is an essential trace mineral that potentiates insulin's action at the receptor level โ meaning it helps your cells respond to the insulin you already produce, rather than requiring the pancreas to pump out more. In women with PMOS (PCOS), where insulin receptors are already less responsive, chromium acts as a metabolic sensitiser. It also helps reduce carbohydrate cravings by stabilising blood glucose fluctuations โ which is one of the most common and difficult daily symptoms of PMOS (PCOS)-related insulin resistance.
Dosage guidance
200โ1,000mcg chromium picolinate daily with meals. Start at 200mcg and increase gradually. Picolinate form is significantly better absorbed than chromium chloride. Works synergistically with inositol and can be combined safely. Not suitable at high doses during pregnancy โ discuss with your provider if planning to conceive.
Section 03 ยท Androgen Excess & Hormone Balance
The hair, the acne, the weight.
There is a reason โ and a response.
"I'm losing hair on my head, growing it where I don't want it, and breaking out like a teenager. No one has explained why โ or what to actually do about it."
Elevated androgens (testosterone, DHEA-S, androstenedione) are responsible for many of the most visible and distressing symptoms of PMOS (PCOS) โ hair thinning, hirsutism, acne, and oily skin. Addressing androgen excess in PMOS (PCOS) requires both reducing androgen production (often driven by elevated insulin and LH) and supporting healthy androgen clearance through liver metabolism and gut health.
One of the most accessible anti-androgenic botanicals
Why it matters
Spearmint contains compounds that reduce free testosterone by increasing sex hormone binding globulin (SHBG) โ the protein that binds testosterone and reduces its activity in tissues. In women with PMOS (PCOS), SHBG is often abnormally low, which leaves more testosterone in its free, active form. Spearmint helps correct this without suppressing androgen production entirely, making it a gentle, well-tolerated first-line botanical for hirsutism and hormonal acne.
Dosage guidance
Two cups of spearmint tea daily (from standardized tea bags) or 900mg standardised spearmint extract daily. Clinical studies used spearmint tea twice daily over 30 days and demonstrated significant reductions in free testosterone. Supplement form is more concentrated and convenient for consistent dosing. Well-tolerated with minimal side effects at standard doses.
For androgenic hair loss and hirsutism โ targets DHT,
Why it matters
Dihydrotestosterone (DHT) โ not testosterone itself โ is responsible for androgenic hair loss (thinning at the crown and temples) and contributes to hirsutism in PMOS (PCOS). Saw palmetto inhibits 5-alpha reductase, the enzyme that converts testosterone into DHT. It works peripherally in the hair follicle rather than systemically, making it a targeted option for hair-specific androgen concerns with a favourable safety profile compared to pharmaceutical 5-alpha reductase inhibitors.
Dosage guidance
160โ320mg standardised lipophilic extract daily with food. Look for extracts standardized to 85โ95% fatty acids and sterols โ this is the active fraction. Allow 6 months before assessing response for hair-related concerns, as the hair growth cycle is long. Can be combined with zinc for enhanced DHT inhibition. Not appropriate during pregnancy.
A mineral with direct anti-androgenic action and profound impact on hormonal acne and hair health
Why it matters
Zinc inhibits 5-alpha reductase (the same enzyme targeted by saw palmetto), suppresses sebum production in the skin, and supports healthy immune and inflammatory regulation in the skin and hair follicle. Deficiency is extremely common in PMOS (PCOS) and worsens androgen sensitivity, acne severity, and hair loss. Zinc also plays a critical role in insulin receptor function โ making it doubly relevant for insulin-resistant PMOS (PCOS).
Dosage guidance
25โ40mg elemental zinc daily with food. Bisglycinate or picolinate forms are significantly better absorbed than zinc oxide. Long-term supplementation above 40mg can deplete copper โ pair with 1โ2mg copper to maintain balance, or ensure your multivitamin contains copper. Do not take zinc on an empty stomach as it can cause nausea.
Section 04 ยท Cycle Regulation & Ovulation Support
Your cycle should
work for you.
"My period comes whenever it feels like it. Sometimes it doesn't come at all. I've been told this is just PMOS (PCOS) โ but I know there has to be something I can do."
Irregular or absent cycles in PMOS (PCOS) are most commonly driven by anovulation โ the ovaries produce follicles that don't fully mature or release. The root causes are almost always insulin resistance and elevated LH (which stimulates androgen production instead of triggering ovulation). Once insulin sensitivity is addressed, these additional supplements can support the hormonal signalling that restores cycle regularity.
Ovulation induction, insulin sensitization, and antioxidant protection
Why it matters
NAC is the building block for glutathione โ your most powerful internal antioxidant โ and is also one of the most clinically studied supplements for PMOS (PCOS) cycle restoration. It improves insulin sensitivity through its own metabolic pathway, reduces oxidative stress in the ovarian environment, and has been shown in multiple trials to support ovulation induction when used alongside or instead of Clomid. It also protects egg quality by reducing the oxidative damage that elevated androgens and inflammatory markers cause to developing follicles.
Dosage guidance
600โ1,800mg daily in divided doses. For cycle regulation alongside inositol: 600mg three times daily. For ovulation support during a medicated cycle: discuss timing with your reproductive specialist. Discontinue 2 weeks before egg retrieval if undergoing IVF. Well-tolerated โ some women note mild GI symptoms initially, which typically resolve within 1โ2 weeks.
For cycle irregularity driven by elevated prolactin or luteal phase deficiency โ use only when the picture supports it
Why it matters
Vitex works by reducing prolactin โ a hormone that, when mildly elevated, interferes with progesterone production and shortens or disrupts the luteal phase. In women with PMOS (PCOS) whose primary dysfunction is elevated prolactin or luteal phase defect (short cycles, spotting before the period, or confirmed low mid-luteal progesterone), vitex can meaningfully restore cycle regularity. It is not appropriate for all PMOS (PCOS) presentations and should not be the first intervention โ address insulin resistance and androgen excess before considering this.
Important notes
Vitex is inappropriate for women with already-normal prolactin and is contraindicated alongside hormonal fertility medications (Clomid, Letrozole, gonadotropins) or dopamine-affecting medications without specialist guidance. Stop at confirmed pregnancy. Effect onset typically 3โ6 months โ this is not a quick fix.
Section 05 ยท Chronic Inflammation
Inflammation is both a cause
and a consequence of PMOS (PCOS).
"I feel puffy, inflamed, and exhausted โ even when I'm eating well. No one has connected this to my PMOS (PCOS)."
Low-grade chronic inflammation is present in virtually all women with PMOS (PCOS), regardless of weight. It worsens insulin resistance, elevates androgens, impairs ovarian function, and drives the fatigue and joint discomfort many women with PMOS (PCOS) experience. Reducing the inflammatory load is not optional โ it is foundational.
Anti-inflammatory, insulin-sensitizing, and cardioprotective โ every woman with PMOS (PCOS) should be on this
Why it matters
Omega-3s reduce pro-inflammatory prostaglandins, lower triglycerides (which are commonly elevated in PCOS), improve insulin sensitivity, and support healthy LH levels. Women with PMOS (PCOS) have a significantly elevated lifetime cardiovascular risk โ omega-3s address multiple risk factors simultaneously: triglycerides, inflammation, blood pressure, and endothelial function. Triglyceride form absorbs 70% better than the ethyl ester form used in most budget brands.
Dosage guidance
2,000โ3,000mg combined EPA+DHA daily โ not total fish oil milligrams. The EPA fraction is most important for PMOS (PCOS) inflammation and triglyceride reduction; choose a higher-EPA product. Third-party tested for heavy metals is non-negotiable. Algae-based DHA is equally effective and appropriate for women who prefer a plant-based option.
For women with PMOS (PCOS) and concurrent endometriosis, fibroids, pelvic pain, or significant systemic inflammation
Why it matters
Curcumin turns off NF-ฮบB โ one of the central switches that drives inflammatory gene expression. Research in PMOS (PCOS) shows curcumin also improves insulin sensitivity, reduces testosterone, improves lipid profiles, and lowers CRP. The key is bioavailability โ standard turmeric powder and inexpensive capsules are largely broken down before they reach the bloodstream. BCM-95 and phytosome forms absorb 6โ8 times more effectively, which is why form matters so much with this particular supplement.
Dosage guidance
500โ1,500mg BCM-95 or phytosome curcumin daily with food. Take with black pepper (piperine) if using a standard curcumin extract โ though BCM-95 and phytosome forms do not require piperine for absorption. If undergoing IVF, pause during the stimulation phase and resume after embryo transfer only with specialist approval, as high-dose curcumin may interact with CYP enzyme pathways.
Section 06 ยท Adrenal PMOS (PCOS) & Stress Regulation
Not all PMOS (PCOS) is ovarian.
Some of it starts in the adrenals.
"My PMOS (PCOS) symptoms get dramatically worse when I'm stressed. I crash after periods of high output. My labs show elevated DHEA-S but 'normal' testosterone. Something isn't adding up."
Approximately 20โ30% of women with PMOS (PCOS) have a predominantly adrenal-driven form โ elevated DHEA-S (produced by the adrenals, not the ovaries), pronounced symptom flares under stress, and often normal or only mildly elevated testosterone. For these women, addressing the HPA axis โ the stress response system โ is the priority intervention, not insulin resistance.
Deficient in most women with PMOS (PCOS) โ involved in insulin signalling, cortisol regulation, sleep, and progesterone synthesis
Why it matters
Magnesium is a cofactor in over 300 enzymatic reactions in the body โ including insulin receptor signalling, progesterone synthesis, and cortisol regulation through the HPA axis. Deficiency is extremely common in PMOS (PCOS) and is worsened by stress, elevated blood sugar, and the diuretic effect of high cortisol. Glycinate form absorbs well and is gentle on the digestive system. Evening dosing supports the parasympathetic nervous system shift that improves sleep quality โ a key driver of PMOS (PCOS) metabolic dysfunction.
Dosage guidance
320โ420mg elemental magnesium daily, taken in the evening with food. Glycinate or bisglycinate forms are the most bioavailable and least likely to cause loose stools. Oxide form (found in most inexpensive supplements) has less than 4% absorption โ it is functionally useless. Magnesium can be taken long-term safely at these doses. Works synergistically with Vitamin D3 โ both are better utilised when the other is replete.
For adrenal PMOS (PCOS), stress-pattern symptom flares, and cortisol-driven cycle disruption
Why it matters
KSM-66 ashwagandha is the most thoroughly studied adaptogen for HPA axis regulation. In PMOS (PCOS), chronically elevated cortisol drives DHEA-S production in the adrenals, worsens insulin resistance, and disrupts the GnRH pulse rhythm that governs ovulation. Ashwagandha meaningfully reduces serum cortisol, supports thyroid function (particularly relevant as subclinical hypothyroidism frequently co-exists with PCOS), and improves sleep quality and stress resilience without sedation.
Dosage & cautions
300โ600mg KSM-66 daily with food, morning or midday. Discontinue at confirmed positive pregnancy test โ insufficient safety data in first trimester. NOT appropriate for women with Hashimoto's thyroiditis or any autoimmune thyroid condition without endocrinologist guidance, as ashwagandha can stimulate thyroid activity and worsen autoimmune reactivity.
Section 07 ยท Gut Health & Metabolic Support
Your gut is part of
your hormone story.
"I bloat constantly, feel heavy after meals, and struggle to lose weight no matter what I eat. I didn't know this was connected to PMOS (PCOS)."
Emerging research consistently shows that women with PMOS (PCOS) have altered gut microbiome composition compared to women without PMOS (PCOS) โ with lower diversity, reduced beneficial bacteria, and increased gut permeability. This dysbiosis worsens insulin resistance, drives systemic inflammation, and impairs the gut's ability to clear estrogens and androgens โ creating a compounding hormonal loop. Supporting gut health is one of the most under-discussed interventions in PMOS (PCOS) management.
Restoring gut microbiome diversity is now considered a meaningful intervention for PMOS (PCOS) metabolic dysfunction
Why it matters
Specific Lactobacillus and Bifidobacterium strains have been shown to reduce insulin resistance, lower testosterone, decrease inflammatory markers, and restore healthier estrogen and androgen clearance through the gut. The gut-hormone axis (sometimes called the "estrobolome") is a clinically recognised pathway โ how well your gut microbiome metabolises circulating hormones directly affects how much gets recycled back into circulation. For women with PMOS (PCOS), a dysbiotic gut can mean androgens stay elevated longer.
What to look for
Choose a multi-strain formulation with at least 25 billion CFU from identifiable Lactobacillus and Bifidobacterium species. Refrigerated formats maintain viability better than shelf-stable in most cases. Take with food, not on an empty stomach. If you are cycling berberine (which has antimicrobial properties), take probiotics on the off-weeks or time them 4+ hours apart from berberine.
"Every supplement here is chosen because the evidence supports it โ not because the wellness industry sells it."
